Chapter Summary

• Context: A herniated disc torácica é menos frequente que as hérnias cervical e lombar, porém sua relação anatômica com a medula torna algumas apresentações particularmente graves. A thoracic kyphosis, o reduzido espaço disponível no canal e características da vascularização medular aumentam a vulnerabilidade neurológica. Muitas hérnias são incidentais e permanecem assintomáticas, enquanto outras produzem dorsalgia, dor radicular, manifestações que simulam doenças viscerais ou mielopatia progressiva. Calcificação, tamanho, localização e eventual relação com a dura influenciam fortemente a complexidade do tratamento. A RM confirma a compressão neural e a TC caracteriza a calcificação. Nos pacientes sem comprometimento neurológico significativo, o conservative treatment pode ser apropriado. Quando há mielopatia ou falha clínica, a escolha cirúrgica é individualizada. Abordagens posterolaterais, anteriores, retropleurais, transdurais, toracoscópicas e endoscópicas coexistem, e o capítulo enfatiza que nenhuma delas é ideal para todos os padrões de hérnia.
• Chapter Objective: Reconhecer as particularidades anatômicas e clínicas da hérnia torácica, compreender sua história natural e os fatores relacionados ao risco neurológico, interpretar RM e TC e selecionar entre conservative treatment e as diferentes alternativas cirúrgicas segundo localização, calcificação, magnitude da compressão e experiência da equipe.
• Uma região anatomicamente críticaA thoracic spine é menos móvel, porém a relação canal-medula e a circulação medular tornam a compressão anterior particularmente relevante. A Tabela 44.1 reúne fatores anatômicos associados ao risco de mielopatia.
• Apresentações variadasDorsalgia é frequente, mas hérnias laterais podem provocar dor em faixa seguindo dermátomos torácicos e simular causas abdominais ou viscerais. Hérnias centrais ou volumosas podem produzir sinais piramidais, alteração da marcha e disfunção esfincteriana. As Tabelas 44.2 e 44.3 e a Figura 44.1 apresentam sistemas de classificação clínica e anatômica. O conceito principal é distinguir localização, magnitude e impacto sobre a medula.
• RM e TC têm funções complementaresA RM caracteriza a relação com a medula e identifica sinais de mielopatia. A TC é essencial para demonstrar se o componente é calcificado ou ossificado, elemento que altera a dificuldade e os riscos da ressecção. A Figura 44.3compara diferentes padrões de hérnia.
• TratamentoPacientes assintomáticos ou sem déficit podem ser acompanhados ou tratados conservadoramente. Mielopatia progressiva e deterioração neurológica favorecem decompression. A escolha da via não deve ocorrer apenas por preferência pessoal. Localização, nível, calcificação, tamanho, anatomia individual e experiência do cirurgião precisam ser considerados. A laminectomy posterior isolada para uma compressão predominantemente anterior é apresentada como estratégia historicamente associada a resultados inadequados.
• Evolução das abordagensA Figura 44.4 demonstra diferentes corredores posterolaterais. Abordagens ventrais e retropleurais podem oferecer acesso mais direto para determinados padrões. Técnicas transdurais e endoscópicas são apresentadas como alternativas mais recentes para casos selecionados. A necessidade de spinal fusion depende da extensão da ressecção, deformidade e integridade estrutural remanescente.
• Clinical Application: Dorsalgia inexplicada associada a sintomas em faixa, alterações de marcha ou sinais de trato longo deve levantar a possibilidade de patologia torácica. A raridade da doença não deve levar à atribuição automática dos sintomas a causas musculares ou viscerais. A RM responde à pergunta “há comprometimento neural?”, enquanto a TC ajuda a responder “qual é a natureza estrutural da hérnia?”. Essa combinação é particularmente relevante porque calcificação e aderência dural influenciam risco e planejamento. Quando não existe mielopatia, a história natural permite frequentemente uma estratégia conservadora. A presença de déficit progressivo exige outro raciocínio. Nos casos cirúrgicos, a via deve permitir trabalhar a lesão com mínima manipulação da medula. Essa ideia é mais importante do que defender uma abordagem universal. Marcação correta do nível, neuromonitorização e antecipação de possível lesão dural constituem elementos de segurança enfatizados pelo capítulo. O desenvolvimento de acesso retropleural, endoscopia e técnicas transdurais representa tentativa de reduzir a morbidade das operações tradicionais, mas não elimina a importância da seleção adequada do caso e da curva de aprendizado.

Why this chapter matters

A baixa frequência da hérnia torácica favorece atraso diagnóstico, enquanto a anatomia da região reduz a margem de segurança quando há compressão medular. O capítulo ensina duas precauções essenciais: não ignorar sintomas neurológicos ou dores atípicas e não transportar automaticamente para a thoracic spine técnicas consagradas em outras regiões. A escolha adequada da via é parte do tratamento e influencia diretamente a possibilidade de decompression segura.

“A hérnia torácica é incomum, mas pode produzir comprometimento neurológico grave. RM e TC devem ser interpretadas de forma complementar, sobretudo para avaliar mielopatia e calcificação. Pacientes sem déficit podem ser tratados conservadoramente, enquanto deterioração neurológica favorece decompression.
